b5fffa12b6
Removing calls that tested if a document was open and replacing them with other validity checks. Documents should be valid and usable if they are created without opening from another source. Some documents like material documents needed extra checks to prevent accessing uninitialized values.

Updated create menu action helper function to find the action to the menu as its parent and automatically insert the action at the desired position in the menu. This fixes a memory leak caused whenever menus were rebuilt but the actions and their callbacks were not connected directly to the menu.
