morphos: implement StackTop to get the stack top from the OS. stack boundaries are detected more precisely now, this fixes stack backtraces with shallow stack, among others

morphos: take two on the Pascal startup code. this version works as a drop-in replacement for the asm one, and doesn't need system unit patching. this will be important when linking against LibC and when the approach gets ported to other Amiga-like platforms

morphos: use MEMF_SEM_PROTECTED flag for semaphore protect our memory pool. this is required for the future threading (concurrent accesses to the same pool from separate threads).

* Implemented late thread manager initialization. Thread manager code is not included to executable if thread stuff are not used. WinCE executable size is reduced by 4KB, Win32 executable size is reduced by 3KB. No testsuite regressions.
